چکیده

RUPTURE iS the usual cause of death in cases of dissecting aneurysm of the aorta in the acute stage. In Sheenan's series of 300 cases (1934) the pericardium was the commonest site of rupture, the pleural cavity being next in frequency. Sheenan described only two cases that survived intrapleural rupture of a dissecting aneurysm, one for 17 days, the other 19 days, and in a review of 505 cases of dissecting aneurysm in the English literature from 1934 to 1958, Hirst, Johns & Kine (1958) describe only six cases that survived for 1 month, of which one survived for 3 months. Post (1941) described the case of a 51-year-old man with multiple sclerosis who survived the rupture of a dissecting aneurysm into his left pleura for 9 weeks before dying from a septicaemia, the consequence of bed sores. Necropsy showed laminated clot at the site of rupture and Post suggested that the leak had been a slow one allowing the patient to survive the rupture. Campbell (1946) described a case of ruptured dissecting aneurysm who survived for 3 months and was shown at necropsy to have died from further rupture. In this case report, the patient remains alive and well 16 months after rupture of a dissecting aneurysm into the left pleural cavity.

Spontaneous aortic rupture early postpartum without trauma or connective tissue disorder.

Rupture of the thoracic aorta is a rare but recognized complication following pregnancy. The common causes of thoracic aortic rupture in the peripartum period are trauma, dissecting aneurysms and saccular aneurysms secondary to systemic connective tissue disease. We report a case of non-traumatic spontaneous aortic rupture in a patient without trauma or systemic connective tissue disease 1 day ...

Drug Therapy for Small Abdominal Aortic Aneurysm

Dear Editor,Abdominal aortic aneurysm is often asymptomatic, less recognized, and causes considerable mortality and morbidity, if missed. The incidence varies from country to country and the occurrence is influenced by modifiable (smoking, coronary heart disease, hypertension, dyslipidemia, and prolonged steroid therapy) and non-modifiable risk factors (increasing age, male gender, and positive...

Aortocaval Fistula Resulting From Rupture of Abdominal Aortic Dissecting Aneurysm Treated by Delayed Endovascular Repair

Aortocaval fistula (ACF) after rupture of an abdominal aortic dissecting aneurysm is a rare emergency situation, which has a high mortality. However, the diagnosis is usually delayed, which increases the difficulties of treatment. We describe a case that successfully delayed use of endovascular aneurysm repair (EVAR) for ACF resulting from rupture of abdominal aortic dissecting aneurysm. We desc...

Chronic Dissecting Aneurysms.

In 1939 East was able to find records of only 27 instances in which the diagnosis of dissecting aneurysm had been made during life. Usually death from complete rupture of the aorta quickly follows the initial dissection; of the 300 cases collected by Shennan (1934) 210 died within a week and survival for more than a year was recorded in only 16 cases. We have, therefore, thought it desirable to...

Turner's syndrome associated with bicuspid aortic stenosis and dissecting aortic aneurysm.

A case of Turner's syndrome is described associated with bicuspid aortic stenosis and fatal rupture of a thoracic dissecting aortic aneurysm. Histology of the aneurysm showed severe cystic medial necrosis. This association has not been previously described in the absence of coarctation. In view of the possibility of surgical repair, dissecting aortic aneurysm should be considered in all patient...
